Records Date of registration, date of cancellation of registration, name and style of home, address, brief description of situation, accommodation for patients, maternity, whether or not in existence prior to 1st July 1928 as Maternity Home or Nursing Home or both, particulars of other business transacted on premises, names of nursing staff and qualifications, proportion of qualified nurses, where the nursing staff are accommodated, names of resident and visiting Physician and Surgeons and their qualifications. Name of person in charge of home and qualifications, number and particulars of 'Alien's employed. Fees charged to patient, particulars of applicant, and particulars of inspection.
