The columns include: full name, address and occupation of injured workman; signature of workman or other person making entry; date entry made; date of accident; room or place in which accident happened; cause and nature of injury. This volume contains three entries only, in respect of William George Peacock (laceration to head, 1943), David John Savage (steel chip in right eye, 1955) and Laurence John Peat (index finger, 1962).
