Index to case sheets 1-2338 (males). At front schedule of forms of insanity as at the time of record [1912], which is useful in understanding the codes used in the volume. A-Z by surname. Column headings = Name, Index No [this is the number needed to find the case sheet], Register No; Schedule Symbol [see schedule at front of volume]; Discharged date; Died date. Some entries note removal to other hospitals, transfers and whether notes are attached to a later file i.e. when a patient returned and new and old case notes were amalgamated.

Access restricted to archive staff as some entries may still be covered by access to medical file rules.
