Bowels (nee Seamarks) of Aspley Heath has informed that some little time ago she registered with you the birth of an illegitimate male child and in error gave the name of the child's father, Judge. She is now desirous of correcting this error and wishes to change the name in the register from Frederick William Henry Judge to Frederick William Henry Seamarks, her maiden name. Will you be kind enough to let me know what procedure is necessary to carry this correction out'.
